Wellfleet partners with Applicant Insight & NoMoreForms™ to provide Agent/Broker Contracting and Background Screening Services for Wellfleet Insurance Company. Please carefully read through this document and complete these next steps within 48 hours to begin the process. Please note each individual form does not open in a separate tab, therefore, we strongly recommend users do not close the form specific windows as this would result in closing the entire NMF application. As a result, data may not be saved and a user would need to re-enter the information in that specific form. Requirements for Creating a Personal Password:
oPassword must be between 8 and 10 characters
oPassword must contain at least one uppercase character
oPassword must contain at least one number or special character

To continue the Agent/ Broker Contracting Process:
1.Carefully read the instructions in the next steps, then click the link in Step 5 below. Complete the electronic contracting forms, authorization and disclosure form.
2.It will take about 10 -20 minutes to complete the forms. If you need to stop during the process, you can save the form you are working on and come back to the remaining forms at a later time.
3.As part of the contracting process, we require a copy of an E&O Certificate. Please have this document available to upload into NoMoreForms™. You will not be able to complete the process without attaching this document to your electronic file.
